For health care professionals who provide care and support for CPA clients there is some information below to assist you to complete Medication Authority Forms.
These documents provide guidance on CPA's requirements and other considerations when completing medication forms for CPA clients, as well as an example Medication Authority Form (MAF) to demonstrate how to fill in the MAF. There are blank CPA MAFs for the medical practitioner to download, fill out for their CPA client and sign.

Medication information for medical practitioners

This document provides background and guidance for medical practitioners regarding CPA’s Medication Policy that we need to follow when giving medication to CPA clients. This includes using simple non-medical language as our staff are not health practitioners, and forms being completed by a medical practitioner so that our staff can give medications to our clients as safely as possible. There are also mock Medication Authority Forms (MAFs) attached, which show the medical practitioner how we would like the MAF to be filled in.

Medication Authority Form (MAF) and Regular Medication and Medication Authority Form (MAF) – PRN/Over the Counter Medication

These are blank forms for the medical practitioner to type the medication orders into. We ask the medical practitioner to print off and sign before giving to our CPA staff or clients. The form can be saved to the medical practitioner’s computer, and then the next time the MAF needs to be reviewed, updated, or medications are added or ceased, it is much easier than rewriting the whole form.
