Transition Compass Delphi Survey

About the study

This study is part of a broader research program aimed at improving the transition from paediatric to adult healthcare services for young people with chronic health conditions, recognising that poorly supported transitions can lead to disengagement from care and poorer health outcomes. There is currently no agreed way to define or measure what makes transition “successful”. Our team has developed draft items for a new outcome/experience measure, and this Delphi survey seeks your input to determine which questions are most important to include.

Who can participate

Anyone who has experienced transition from specialist paediatric to specialist adult care. This includes:

  • young people (18-24yrs) with chronic illness,
  • parents of young people with chronic illness,
  • paediatric healthcare professionals that transition young people, and
  • adult healthcare professionals that receive young people
