A recent report from Adara Group, supported by a CPA Research Foundation grant, shows how the Hospital to Home (H2H) program in Uganda, a newborn follow-up and early intervention model, has been instrumental in bridging the gap between hospital care and community support to make a real difference.
Preventing complications around birth, including birth asphyxia, is a critical focus of Adara’s work. In Uganda, birth asphyxia contributes to around half of all neonatal deaths and is a major cause of long-term neurodisability. Adara’s H2H program, is focussed on all aspects of antenatal support and safer births, improving newborn care, neuroprotective practices and early identification of neurodisabilities.
With CPA support, Adara successfully adapted the H2H programme for Uganda’s public health system and launched a pilot at Nakaseke Hospital in 2025. Across two hospital sites, more than 900 high-risk babies were enrolled, with 98–99% survival rates, alongside improvements in breastfeeding, growth and vaccinations.
A key focus of the CPA-funded work has been training and empowering Community Health Workers to recognise early developmental concerns and refer babies for further assessment and support. Many babies showing early signs of neurodisability were identified and referred into early intervention pathways giving families the chance to access support sooner, when it can make the greatest difference.
This reflects CPA’s belief that sharing evidence globally helps to strengthen capacity within local systems creating more of a sustainable impact.
By supporting partners like Adara, CPA is helping ensure that advances in newborn care, neuroprotection and early identification are not limited to high-income countries but shared where the need – and potential for impact – is greatest.
Adara is now working in partnership with Uganda’s Ministry of Health implement the H2H model nationally. This CPA-funded work demonstrates how global collaboration can change outcomes for babies, families and communities worldwide.
