Cerebral Palsy Alliance (CPA) will listen to your feedback and resolve problems quickly and fairly. Information about making a complaint about services is available here.

CPA will provide you with a Service Summary and you may request further details at any time.
When you/your child transitions to alternative funding arrangements for services, a new service agreement will be developed with you.
CPA reserves the right to claim funding for Supported Independent Living (SIL) services delivered on either a weekly or hourly basis at our discretion.
You are required to follow any staff requests in relation to health, safety and safe lifting practices.
Any photographs or videos you take of service/therapy sessions are for private use only. You agree not to share these on social media or in any other way.
CPA reserves the right to adjust fees in line with changes to the NDIS benchmark pricing (reviewed annually) to enable us to maintain standards of excellence.
CPA will issue regular invoices and/or statements of supports delivered. Payment for NDIS self‑managed or privately funded services is required within the timeframe specified on your invoice. Where services are funded directly by the NDIS (Agency Managed) or through a Plan Manager, it is your responsibility to ensure sufficient funds are available to cover services delivered. Please notify CPA as soon as possible if there are any delays in funding. Please note that if NDIS funding is insufficient to cover the agreed services, you will be responsible for paying any shortfall.
CPA aims to give you notice of cancellation as per our cancellation guidelines and asks that, where possible, you give notice as per the guidelines for changed or cancelled services.
CPA reserves the right to charge a cancellation fee as per our cancellation guidelines. Note: This does not apply to accommodation services.
Services may be suspended if you repeatedly cancel appointments, do not attend, or do not pay for services that have been invoiced (please see cancellation guidelines).
If we need to end your services, CPA will give you at least 14 days’ written notice. For accommodation services, we’ll give you at least 90 days’ notice.
If you are using your NDIS funding:

The supply of supports under this Service Agreement is a supply of one or more reasonable and necessary supports specified in the statement of supports included, under subsection 33(2) of the National Disability Insurance Scheme Act 2013 (NDIS Act), in the participant’s NDIS Plan currently in effect under section 37 of the NDIS Act.”

This Agreement supersedes all prior terms and conditions between you and CPA relating to the services covered by this Agreement.

General consents

I consent to receive services and information about services from CPA and understand that electronic and paper records will be maintained about the services provided to me.
Records will be maintained and stored in accordance with the Australian Privacy Principles and other relevant Privacy legislation.
Records will be accessed by staff who are bound by privacy and confidentiality policies in accordance with legislation.
My consent will be obtained if information needs to be released to an agency, or if such other agency requests information about me, unless sharing of information is required by law.
De-identified information may be used for research projects approved by CPA, or shared with government departments under some funding agreements.
If I change my mind about this consent, I may cancel it at any time in writing.
